
The ENcourage Foundation® is a nonprofit foundation supported by Amgen and Wyeth that helps make ENBREL® available to qualifying patients who could otherwise not afford treatment. To date, the ENcourage Foundation® has helped more than 24,000 qualifying patients receive treatment with ENBREL®.
Enrolling in the ENcourage Foundation®
The ENcourage Foundation® provides assistance for patients receiving ENBREL® who reside in the United States, are uninsured, and demonstrate financial need as determined by the Foundation’s income criteria.
Enrollment process: Patients can enroll directly in the ENcourageFoundation® by completing an application packet. The patient must complete and sign the Patient Enrollment Form and submit supporting income documentation. The patient’s physician must validate and sign the Prescription Form. The ENcourage Foundation® will review the information, determine eligibility, and enroll qualified patients.
Enrollment period: Eligible patients typically are enrolled in the ENcourage Foundation® for 12 months.
Re-enrollment: Patients are eligible for assistance as long as they continue to require treatment with ENBREL® and continue to meet program eligibility criteria. To re-enroll in the ENcourage Foundation®, patients must complete a new application packet prior to the end of their enrollment period. The ENcourage Foundation® will send out a re-enrollment letter and application to patients two months prior to the enrollment end date.
Receiving ENBREL from the ENcourage Foundation®
ENBREL® will be shipped directly to the patient through a licensed specialty pharmacy, RxCrossroads®, unless otherwise requested by the physician.
