
The Safety Net Foundation is a confidential patient assistance program that has helped over 250,000 qualifying patients receive treatment with Amgen products. Serving uninsured patients by ensuring that the vital medicines they need are within their reach, The Safety Net Foundation makes Amgen products available to those qualifying patients who could not otherwise afford them.

How Are Patients Enrolled in The Safety Net Foundation?
There are two basic steps to enroll a patient in The Safety Net Foundation. Specific instructions are found in all of the patient enrollment forms. The forms are usually completed by the sponsoring facility.
For Aranesp®, EPOGEN®, Neulasta®, NEUPOGEN®, and Vectibix®:
For Sensipar®:
The application process for Sensipar requires a completed Patient Enrollment Application (Form B) and Prescription Form (Form C).
For Nplate® Patients:
Nplate® is available only through a restricted distribution program called Nplate® NEXUS (Network of Experts Understanding and Supporting Nplate and Patients) Program. Please contact the NEXUS Program at 1-877-Nplate1 (1-877-675-2831) for more information about this Program.
Enrollment Specifics
Enrollment year: Eligible patients are typically enrolled in The Safety Net Foundation for up to one calendar year.
Re-enrollment: Patients are eligible for assistance as long as they continue to require treatment with one (or more) of Amgen’s products and continue to meet program eligibility criteria.
