The Amgen website will not be accepting applications for 2009 programs/events after 12:00 am PST Thursday, October 1, 2009. Any applications submitted after this date will not be guaranteed review and processing for the 2009 program. Applications for the 2010 program will continue to be accepted and reviewed, but please note that applications received for the 2010 program may not receive responses until sometime in 2010. For questions, please call the helpline at 1-866-442-6436.

Q: Where do I submit Healthcare Donation requests?
A: Please email or fax your supporting documents and the printout of the completed online application form to HCCDonations@amgen.com or (888) 272-5510.

OR

Mail the complete unstapled application packet to:

Q: How will I know that my donation request has been received?
A: A confirmation email will be sent to the requestor when the supporting documents required to complete the application are received.

Q: How can I contact Amgen if I have questions about submitting a request?
A: If you have any questions about the submittal process or status of your request, you may contact the Amgen hotline at 866.44AMGEN (866.442.6436).

Q: If I complete the application correctly and provide all applicable documentation, can I assume that my donation request will be approved?
A: No, please do not assume that a request will be approved.  When a decision is finalized, Amgen will communicate approval or denial of the request in writing directly to the requestor.  Verbal approvals by Amgen employees are not authorized.  NOTE: Amgen support of a program should be acknowledged only if an Amgen approval letter has been received by your organization.

Q: What is the expected turnaround time for disposition of the donation request?
A: Please allow sixty (60) days for review and processing of the request.  If it is determined that disposition of a request may exceed sixty days, Amgen will contact the requestor.

Q: Is there a limit on the number of donation requests an organization can submit?
A: No, there are not restrictions on the number of requests that can be submitted, however duplicate requests will be rejected.

Q: The event occurred last week, can I still submit a donation request?
A: No, Amgen cannot fund requests for expenses already incurred by the recipient (i.e. donations must be funded prospectively).  Amgen requires all requests to be submitted at least 60 days prior to the required funding or event date.

Q: I have a copy of a 2003 W-9 form.  Is this the current version?
A: No, the current downloadable W-9 form can be found on the IRS website.  Amgen will only accept the current version of the form signed and dated.
